Отзыв о книге:

Osteopathic Medicine Recall
Nicholas J. Caputo, DO
Alliance Hospitalists Phoenix, Ariz

Osteopathic Medicine Recall, edited by Andrew D. Mosier, DO, and Dai Kohara, DO, is yet another book in the "Recall" series of reference aids for physicians. This book, like many others in the series, assumes that the reader has a fundamental understanding of the material—in this case, osteopathic manipulative medicine.
The book is arranged in a concise and logical question-and-answer format, similar to the methods used during daily rounding between instructors and students. This format challenges the reader in a systematic way that enables memory stimulation and information retention. The condensed format also allows for effective time management, helps the osteopathic physician quickly brush up on fundamentals, and fortifies existing knowledge. This text could easily be referenced before initiating an osteopathic rotation or examination.
Osteopathic Medicine Recall is based on the osteopathic principle of understanding and treating the human body as a whole unit—rather than concentrating on isolated problems and incomplete solutions. Each topic in the text is discussed as it relates to the entire body, emphasizing that an osteopathic physician must understand the interactions among all the body systems with the ultimate goal of returning corporeal homeostasis and harmony.
The book consists of 14 chapters. The first chapter is a review of the basic concepts of osteopathic medicine. Among the many topics covered are allostasis, TART (tissue texture abnormality, asymmetry, restriction of motion, tenderness), myofascial release, passive and active motion, and Fryette's laws. The text provides high-yield information on these concepts and discusses commonly tested data. Each chapter builds on the information presented in previous chapters.
Chapters 2 through 5 delve into a review of the axial and appendicular spine, concepts that are unique to osteopathic medicine. General topics in these chapters include the spine, rib cage, and sacrum; the upper and lower extremities; the autonomic nervous system; and the lymphatics. Numerous questions and challenges are posed in regard to these topics. For example:
• What is the clinical significance of the articular pillars?
• List the three components of the spinal reflex.
Chapters 6 through 12 are dedicated to specific manual techniques used in the delivery of osteopathic manipulative treatment (OMT) with the intention of restoring the body's homeostasis. The OMT procedures covered include articulatory techniques and myofascial release; counterstrain; facilitated positional release; ligamentous articular strain; muscle energy; high velocity, low amplitude thrust; and craniosacral techniques. Examples of questions included in these chapters are:
• What are the indications for articulatory techniques?
• When is a crossed extensor reflex used in muscle energy?
The book concludes with chapters on Chapman reflex points and the Travell trigger points.
Each topic in the book is referenced in the event that the reader would prefer to examine a more in-depth explanation at a later time. The referenced materials are exquisite and include many of the osteopathic medical profession's "bibles"—the "classics" that should be on required reading lists at all osteopathic medical institutions.
Another valuable feature of this book is that the editors have included a number of illustrations that can help the reader in comprehension and memory retention. There are clear, useful diagrams, for example, on the three types of rib motion and on the various tender points.
I must admit that I sometimes felt as though I was cramming for an examination on OMT while reading this book. It jarred loose many memories of past times in the OMT lab, huddled around a table with a group of classmates to review key concepts of osteopathic medicine, reading through the steps to perform particular techniques, referencing illustrations, and then trying to perform the techniques correctly on an understanding and patient volunteer.
Osteopathic Medicine Recall would be most effectively used as a quick reference aid to help an osteopathic physician review the core concepts and principles of osteopathic medicine. The book is very useful for refreshing one's understanding of these concepts and principles, but this text should not be used as a substitute for reading any of its referenced materials.
The book does have some shortcomings. The information provided is rudimentary and, at times, has the feel of a drill sergeant by being too rote. Some information is presented in a disjointed manner within the chapters, with questions and answers on similar topics separated by other, unrelated questions and answers. In addition, the graphics on the book's cover are abstract and generic, providing no visual identity for osteopathic medicine.
It is also disappointing that the editors are not listed as "DOs" on the front or inside cover of the book. Their academic degrees appear only on the book's back cover. I realize that this criticism may be considered "nit-picky" by some, but I am proud of my identity as an osteopathic physician. I believe that our profession does not receive enough publicity or recognition. Perhaps the editors can rectify this oversight in the next edition.
Overall, Osteopathic Medicine Recall has many nice features, including its portability, the concise format it is written in, helpful references, high-yield information, and a logical presentation. The book is a manageable read, well organized, and comprehensive. Above all, the book focuses on core concepts in osteopathic medicine. I wish there had been such a resource available when I was in osteopathic medical school.

Travell.

The Effect of Osteopathic Treatment of Cesarean Section Scars on Pelvic Muscle Strength

Georgio Trimarchi
Research Proposal Presented to the Toronto Jury
March 2, 2008

ABSTRACT

The objective of this research is to determine the nature of myofascial release on transverse cesarean section scars and whether the use of boney anchors for integration (lumbar and sacrum) will alter the muscle strength of specific pelvic muscles.

In this study, the strength of five specific pelvic muscles will be measured for each subject using the Lafayette instrument—a muscle-strength gauge dynamometer.

Sample power statistical software (SPSS) has been used to estimate the sample size needed in order to have an 80% chance of finding a (true) statistically-significant difference in muscle strength between the Control and Treatment period in the study. As a result, it has been determined that a sample of 26 subjects is required for this study.

The Control and Treatment groups are to be made up of women who are between 20 and 45 years old and have had transverse cesarean section scars for more than twelve months. The participants will be volunteers recruited from the local community.

The assessment of muscle strength and treatment of scars will be administered in a professional therapy centre by a fitness consultant and an osteopath graduate student, respectively, over a sixteen-week period.

The desired results of this study are to obtain a deeper understanding of the effects of cesarean section scars on pelvic muscles specifically and the female body in general, and how women might benefit from osteopathy.

Product Description
Developed in the late ’70s by French osteopath Paul Chauffour, Mechanical Link is a gentle manual therapy that encourages the balance of tensions in the fascial system—that complex web of tissue that interconnects and affects all other body systems. It spreads throughout the body uninterrupted, providing physical stability while also allowing flexibility and mobility. Based on the principle that traumatic stress affects the interconnecting tissues of the body by forming patterns of tension called lesions, Mechanical Link therapy has successfully treated fibromyalgia, migraines, asthma, and other conditions. Extremely popular in Europe, it is rapidly gaining adherents in North America. This book, complete with 44 black-and-white photographs and 20 color illustrations, is a comprehensive manual for diagnosing and treating patients.

Mechanical Link therapy is guided by the body’s own wisdom about its unique needs. The work stimulates to the body’s self-corrective responses, promoting normal mobility, tissue tone and posture. Mechanical Link brings tension into equilibrium and allows the body to return to optimal functioning ability, so all its systems can improve—including the immune system.

Mechanical Link helps alleviate a range of illness, pain and dysfunction, including:
•Fibromyalgia
•Indigestion
•Migraine Headaches
•Premenstrual Syndrome
•Asthma
•Chronic Fatigue
•Motor-Coordination
•Impairments
•Chronic Neck and Back Pain
•Central Nervous System
•Disorders
•Emotional Difficulties
•Temporomandibular Joint Syndrome (TMJ)
•Stress and Tension-Related Problems
•Orthopedic Problems

DOES OSTEOPATHIC TREATMENT INFLUENCE THE HORMONE
LEVEL OF HYPERANDROGENAEMIC INFERTILE WOMEN?

Регулирует ли остеопатическое лечение гормональный уровень инфертильных женщин андрогенного типа?

By Sabine Heim
A-6850 Dornbirn
Part of requirement for M.Sc. (Osteopathy)
Donau-Universitat Krems
Abteilung fur Komplementarmedizin
December 2007

Abstract
Backround: In Europe about 14% of the couples have difficulties to conceive. In about 25% the reason for infertility is found in the hormonal system. A main pathology of the female hormonal system is hyperandrogenaemia. On the one hand, an androgen excess leads to distortions of the menstrual cycle, oligo-/amenorrhoea and anovulation. On the other hand women suffer from the apparent external changes due to hyperandrogenaemia: hirsutism, acne, alopecia and android fat distribution.
Research question: Does osteopathic treatment influence the hormone level of hyperandrogenaemic infertile women?
Study design: with-in subject design
Methods: A pilot study comprising 10 test persons was conceptualised, but only a case series of 3 subjects could be gathered. Appropriated and reasonable in- and exclusion criteria were applied.
Each woman got 6 treatments every 2 weeks. Blood samples (testosterone, LH, FSH, TSH) were taken before, during and after treatment including an observation period of at least one month during which no treatment occurred. Additional questionnaires were filled in by the subjects before and after the treatment period.
Results: The testosterone level of two women deduced to a normal value. The one of the third woman remained unimproved, but she became pregnant after 4 treatments. Of note, this woman could not be considered as infertile referring to the definition of the WHO (“Infertility: failure to conceive after at least one year of unprotected coitus”). The LH-FSH-ratio was normal (<2) throughout all measurements in two patients. Only in subject 3 the LH-FSH-ratio was abnormally high (3.4), and normalised after three treatments and remained normal after further three treatments. It should be noted that the same patient had shown normal LH-FSH-ratio two years before. TSH values were normal in all subjects at the beginning of the study, suggesting normal thyroid
functioning.
Discussion: Due to the problem of the recruitment of infertile hyperandrogenaemic women, only this small number of 3 test persons could be recruited. None of these women with pathological elevated testosterone level was infertile. Blood measuring should have included the SHBG level (sex hormone binding globulin) in order to estimate the free testosterone level as only the unbound free testosterone can exert a
hyperandrogenaemic effect.
Conclusion: The results of these three cases after osteopathic treatment do not allow any statement on the effectiveness of osteopathic treatment on the hormone level of hyperandrogenaemic women. Further studies are needed to be conducted on a large scale. These studies should be proceed in two steps: preferably first including hyperandrogenaemic women and afterwards hyperandrogenaemic and infertile women. If once it is proven that osteopathic treatment reduces testosterone levels, infertile hyperandrogenaemic women will be recruited
more easily.

The Meaning of Tensegrity Principles for Osteopathic Medicine

Значение принципов Тенсегрити в остеопатической медицине

Carsten Pfluger

October 2008
Master thesis
for the degree “Master of Science” in Osteopathy
submitted at the Donau University Krems
and the Vienna School of Osteopathy

Abstract

This study deals with the significance of tensegrity principles for osteopathic medicine. The central questions are, whether and in which way these principles can be applied in osteopathy and which scientific basis the application has.

The first part of the study, on the one hand, deals with the development of the tensegrity principles and their application in architecture as well as the transfer of these principles to biological systems in the micro- and macroscopic area with the help of basic research. The understanding of this basis is essential to assess the transfer of these principles in osteopathy. On the other hand, a literature analysis is carried out and it is investigated in which form these principles are transferred to osteopathic medicine. In doing so one concentrates on the quality of the particular sources. In the second part of the qualitative study a critical discussion on the application of the tensegrity principles in osteopathy takes place. In the course of an expert interview, six experienced osteopaths are asked for their opinion on these principles.

It is only these expert interviews that make a critical debate with the tensegrity principles possible, as in literature such a critical discussion does not take place.

Knowing hands converse with an expressive body -
An experience of osteopathic touch

Seth Consedine

A thesis submitted in partial fulfilment of the
requirements for the degree of
Master of Osteopathy, Unitec New Zealand, 2007

Abstract

Aim
The aim of this phenomenological study was to examine and describe the patient’s experience of touch during a consultation with an osteopathic practitioner.

Background
Touch is the process by which one person physically contacts another individual and as such plays an integral role in osteopathic practice. Osteopathic practitioners use touch in almost every facet of care including examination, diagnosis and treatment. Although this form of communication is intrinsic to osteopathic practice, there has been no research undertaken to date that qualifies the experience of osteopathic touch within a treatment session.

Methods
Three local osteopaths identified five potential participants from their own practice. Data were recorded during five face-to-face semi-structured interviews that were conducted by the primary researcher. Interviews were transcribed and the textual data was analysed using the principles of van Manen’s (1997) hermeneutic phenomenology. Using this approach, significant themes were identified in the data and a description of the lived experience of osteopathic touch emerged.

Results
Three phenomenological themes were identified in the data and broken down into nine constituent subthemes. Theme [A] The Process – a physical interaction, drew attention to the physical patient-practitioner relationship and the physical and psychological implications of this interaction. Theme Profession alism – a practitioner’s responsibility explored the role of touch in developing professional boundaries and emphasised the patient’s expectations surrounding the physical contact, while Theme [C] Reassurance – a therapeutic necessity examined the role of touch in reassuring the patient about their presenting complaint and the practitioner’s medical knowledge and technical skill.

Conclusions
The experience of touch for the participants in an osteopathic session is one of care and security. For the participants touch plays a critical role in the therapeutic relationship by supporting and validating their experiences and by communicating practitioner humanism and professionalism. Furthermore, the experience of touch was identified as a critical component of the therapeutic interaction.
Indications for further research and implications for the osteopathic profession are discussed.

Three leaves of Glenard
Examination of a functional aspect

An Interexaminer- and Intraexaminer Reliability Study

Abstract

A look at the research methods applied in manual and osteopathic medicine quickly shows that the results of the interexaminer- and intraexaminer reliability often are inadequate.
Based on our research, a flaw in the verification of examination methods of visceral osteopathy can be described. In order to clarify this aspect of osteopathy, it is the aim of the present study to assess a method of palpatory examination of visceral osteopathy.
This examination method focuses on two aspects, the evaluation of abdominal
tone and tension-conditions, and is reviewed in relation to its interexaminer- and intraexaminer reliability. The anatomical basis for this test is "the tension-model of the three leaves of Glenard", a functional construct which had been described by the French surgeon Glenard Franz in his book "Les ptoses viscerale" (1899). This model is adopted in the concept of visceral osteopathy and forms the base of many osteopathic textbooks.
At the time of our research, no scientific study could be identified that verifies this model and the statements derived from it. Thus it is the task of this study to testify the tension modell described by Glenard (1899, p. 535 ff) by means of the tone and tensiontest.
The basis of this survey is an inter- and intra-reliability design with repeated measurements to be able to testify the efficency of assessing the tone and tension of the abdomen.
The structural analysis of the examination results is done by means of Cohen's Kappa index.
Before the examination, all osteopaths were trained with the objective of achieving a coordinated test-performance and interpretation. In the subsequent examination, seven osteopaths twice analyzed 33 patients. In the second run, the osteopaths carried out the examination wearing sleeping masks to prevent them from optically recognizing their patients. To assess tone and tension, the criteria hyper / normo / hypo were introduced and documented in a specific examination form. Additional assistents were appointed to document the findings and to ensure a smooth test run.
In general, the results of an assessment of the tone are more reliable (moderate reliability) than an evaluation of the abdominal tension (poor reliability). In total, it must be stated that the reliability of this test is too low.
Based on the results of this survey, a general trend in manual medicine can be
confirmed. This trend points to the fact that in terms of an average rating, the findings of interexaminer reliability are less reliable than those of intraexaminer reliability.
A final assessment of this examination shows that the scientific investigation of both the "tension model of Glenard" as well as its use in visceral osteopathy is insufficient.
The present study is criticizing this situation and would like to suggest a further exploration of this revealing aspect of visceral osteopathy.

Description

Oseopathy is a science with possibilities as great as the magnitude of the heavens. It is a science dealing with the natural forces of the body. We work as osteopaths with the traditional principle in mind that the tendency in the patient's body is always toward the normal. There is much to discover in the science of osteopathy by working with the forces within that manifest the healing processes. These forces within the patient are greater than any blind force that can safely be brought to bear from without. - William Garner Sutherland, D.O.

What is Potency?

Exploring Practitioners’ Experiences of the Phenomenon
of Potency in Osteopathy in the Cranial Field

Helen Harrison

A research project submitted in partial requirement of the degree of
Master of Osteopathy, Unitec Institute of Technology, 2009

Abstract

This study explores the phenomenon of Potency as experienced by five practitioners of Osteopathy in the Cranial Field. Phenomena can be elusive. The aim of investigating Potency, as a lived experience, is to gain an in-depth knowledge that will help to deepen understanding to help guide osteopathic practice. In recognition of the increasing trend towards Evidence-based Practice, this study will contribute to the emerging body of research which osteopathy needs to support its position as a mainstream healthcare provider. The exploratory nature of this study fits the qualitative research design and is a hermeneutic phenomenological study guided by the ideas of van Manen. Practitioners for the study were selected using purposive sampling. The data were collected from the five participants by conducting individual interviews.

The aim of phenomenological research is to uncover the themes that give a phenomenon its uniqueness and thereby contribute to a greater and deeper understanding. Thematic analysis identified two themes each comprising sub-themes: Practitioners’ Journey and Essences of Potency. The practitioners’ ability to understand the phenomenon of Potency constitutes a journey comprising many influences. The practitioners appeared to be influenced by many experiences occurring before, during and after osteopathic education. The experience of understanding Potency evolved with the phenomenon having qualities that empowered the practitioners to continue on their journey. This study concludes that the practitioners’ experiences of Potency appear to depend on the context of the practitioners’ reality.

Practitioners’ perceptions of Potency start and continue to evolve in their experiences of Potency. However, these perceptions included Potency as an expression of the Breath of Life. Sometimes this is experienced visually as light and at other times experienced as energy. It appears Potency emerges from stillness, a place of harmony and returns the organism to its originality and health.

The study places the phenomenon of Potency in its professional context by discussing current beliefs and experiences of Potency in the osteopathic literature. The purpose of this study is to contribute to an emerging body of literature on osteopathic phenomena. The study provides suggestions for future research, and gives implications for osteopathic practice and education.
